Near North Health in Chicago, Illinois is seeking an Executive Director of Population Health to lead a singular organization-wide population health system that turns clinical standards into reliable care-gap closure and measurable outcomes. Reporting to the CEO and serving on the executive leadership team, this leader will bring population health strategy, value-based care, care coordination, community programs, clinical informatics, and data-enabled workflows into one coordinated operating model. 

Clinical standards, protocols, and the clinical definition of a care gap remain the responsibility of the Chief Medical Officer. This position owns the systems that operationalize those standards across the population. 

The strongest candidate will be a systems-minded healthcare leader with at least 10 years of experience, including five years leading population health, value-based care, care coordination, or an equivalent cross- cutting function. This person will bring clinical fluency, FQHC and safety-net knowledge, respect for governed data, and the ability to lead both through direct authority and influence.
